Replacement Match Tickets
It is requested that Members take all necessary precautions to protect their event tickets and keep them in a secure place.

In the event that a Members' ticket/s or ticketbook is lost or stolen the Member is to complete the Lost Ticket Form, located below, and return it to the Medallion Club.
A replacement fee of $20 (incl GST) per game applies, regardless of the number of tickets lost for that game.

Replacement ticket requests must be received prior to 3pm on Fridays for weekend events. For mid-week events replacement ticket requests must be received prior to 3pm on the event day. Email notification will not be accepted.

Lost Smart Cards
In the event that a Member's smartcard is lost or stolen, the Member is to complete the Lost Smartcard Form, located below, and return it to the Medallion Club. A replacement fee of $50 (incl GST) applies per smartcard.

Replacement tickets will be issued until the replacement smartcard can be issued.

Lost AFL Medallion Cards
In the event that a Members' AFL Card is lost or stolen, the Member is to complete the Lost AFL Card Form, located below, and return it to the Medallion Club. A replacement fee of $20 (incl GST) applies per card.

Please allow for a minimum of 48 hours for AFL Cards to be replaced.
